Our dentist, Dr. Steven F. Puffer, and our team know that your time is valuable and that you want to maximize your time at our office. To spend less time in our waiting room and more time with our team creating your new smile, please complete and submit your forms before your dental appointment in Nacogdoches, Texas.

Patient Forms & Policies
We ask that you arrive 10 minutes prior to your first appointment. Our team will use this time to validate your insurance, create your electronic chart, build your patient profile and talk with you about your medical history. Please bring any medicine you are taking or a current list of your medications, including any vitamins or supplements. Please let us know if you have a medical condition, including any allergies or pregnancy. We will also need to know the reason for your visit. Keep in mind that any fees, including co-pays and out-of-pocket expenses, are due on the day of your appointment.

Your First Visit
When you visit our Puffer Dental, we will do more than examine your smile. This is our chance to get to know you and create a relationship with a strong foundation that we hope will last a lifetime. Our dentist will speak with you in-depth about not just your dental needs and goals for your smile, but your medical history. This information is critical to helping Dr. Puffer create your personalized care plan.

Tardiness & Rescheduling
We want to ensure that our patients’ visits with us are as timely and convenient as possible. If someone is late, unprepared or is not scheduled for a visit, it can create problems for patients who arrived on time, are ready to talk with our team or have scheduled their appointment ahead of time.

We understand that the unexpected happens and that you may not be able to keep your appointment. Whenever possible, please call our team to cancel or reschedule. If you are running at least 15 minutes late, please contact our office and let us know. If you do not call our office and do not keep your appointment, you will be charged a $25 fee.
